Unlimited Ink Notary can assist you with your Advanced Health Care Directives and Power of Attorney needs within the State of Arizona. The State of Arizona offers its residents an Advanced Health Care Directive and Power of Attorney option that only needs to be Notarized and/or witnessed.  You may download and print the documents below and schedule your appointment or select to have your Notary Print these documents and bring to your appointment.

  • Arizona also offers its residents additional forms like:

    • A General Power of Attorney​ which can be made durable if selected when filling out the document. A General Power of Attorney has a beginning and end date set at the time of signing and a Durable Power of Attorney ends upon the principle's death or revocation.

As a precaution, we encourage all clients to obtain multiple original copies, at the time of signing. We encourage this because all too often when you need to use these documents the receiving parties may need to hold these documents for several days and you may need them in more than one place. 

Please note: If you are not filling these forms out for yourself it is important to know the following before scheduling the appointment:
 

  • You may fill out the documents ahead of your appointment but do not sign them until you are in the presence of a Notary Public.

  • The signer must be aware you are sending a Notary Public to them, what the document(s) are for, and agree with you ahead of booking the service they will be willing to sign the document. No refunds will be issued if the signer is unaware or refuses to sign the documents for any reason. 

  • All signers signing Notarized documents must be coherent. Notaries are not allowed to proceed with signings if a signer is heavily medicated and/or not currently in sound mind and body. It is best to schedule your appointment when you know the signer is awake and alert. 

  • Our booking form below automatically selects for you how many Notarizations are needed; however, you will be responsible for ensuring the witnesses are in place for your signing. Witnesses must be 18 years of age, possess a State ID or Driver's License, and must not be named in the documents. If you are signing these documents at a hospital or jail, you must ensure the facility staff is allowed to witness. 

  • If you are not able to provide witnesses, you may want to schedule your appointment at a buy's coffee shop or restaurant where policemen frequent (like Starbucks or Chick-fil-a) to as a policeman to act as a witness. Or you may purchase additional witnesses at all locations except for jails as they will only allow one Notary in with the inmate at a time.

  • After you appointment you will want to register a copy of the documents with Contexture. This is not a service that is a responsiblity of the Notary Public.
